Revised CMAG Bi-Monthly Funding Options
Dear CMAG Members,
Please find attached a number of funding options which can be accessed by case managers to meet the needs of their clients, that Donna Wagon, Team Leader, Disability Case Management Services, Melbourne Citymission, in 2007 presented during a CMAG Bi-Monthly Forum.
I have revised these funding options, nominated by Donna, and the attached information is up-to-date as of this month - September 2009 (note the date at the base of the document for future reference).
I have also attached information provided directly from The Ian and Nell Clark Encouragement Fund which includes the eligibility, process and applicaiton form.
